MEMBERSHIP RENEWALS 08-09
A 2008/09 MEMBERSHIP RENEWAL PACK has been mailed to all current members.
The details on the renewal form are those recorded in the SLSA database (SurfGuard). It is an SLSA requirement and therefore important, that members CHECK ALL MEMBERSHIP DETAILS and MAKE CHANGES OR PROVIDE INFORMATION where information is incorrect or missing. Club keys number is 2007/08 badge number and should be ignored.
The Renewal Pack comprises five (5) documents. Of these, *three documents are to be returned with full payment.
- Application for Membership (renewing) and SLSA Membership Declaration - all sections on the front and back of this form are to be checked, completed, signed and returned
- Child Protection / Prohibited Employment Declaration (PED) to be completed, signed and returned by members15yrs and over
- Offer to Assist form to be completed, signed and returned
- Club Censusto be completed and returned by 1 July 2008. A letter of explanation from the President accompanies the census.
- Nomination for Office for 2008/09 - no longer required to be returned
A calendar of events and functions, etc is also included in the pack.
*It is a condition of membership that documents 1 to 3 are returned with full payment including locker fee where applicable. Please complete, sign and return the required forms with payment (schedule of fees is included in the pack - see also end of this page) by 15 June 2008 to be eligible to vote at the Annual General Meeting. Please print clearly. To reduce postage costs, please include a stamped envelope.
By applying for membership of Manly Life Saving Club, you are agreeing to accept and abide by the respective Rules, Regulations and Resolutions of Manly Life Saving Club. Members 60yrs and over are eligible for 50% discount on the membership fee (only).
Incomplete applications, illegible and/or unsigned documents cannot be processed and will be returned. If we have not received your completed, signed forms and payment by 1 October 2008, a rejoining fee will apply.
Membership applications (including renewals) are submitted to the first Management Committee following receipt of completed forms and payment. If an application is approved, the membership badge and receipt will be posted to the member (please include a stamped envelope).If for any reason membership renewal is refused, the fees paid will be refunded in full. C graded patrolling members are required to include a letter of commitment to perform at least 30 patrol hours during the 08/09 season. FINAL Patrol hours and gradings appear at the end of this page.

Members wishing to make a donation with their fees are asked to include the donation amount with fee payment. A receipt will be mailed as soon as possible with the 2008/09 badge.
